Get Involved

By participating in any Sunday Strong Class, I understand and agree to the following terms:

1. It is my responsibility to decide with my doctor whether any class and/or class activity is safe for my body.
2. I know that there is a risk of injury and I accept that risk. I may have to get medical care due to injury from participating in any Sunday Strong class, including concussions. I may have to wait until a doctor gives me permission to return to class. I assume any risks for my participation and hereby waive, release and discharge Sunday Strong from any liability related in any way to my participation.
3. I am responsible for telling my coach about any conditions, health concerns, injuries or limitations that may negatively affect my class participation.
4. I can follow rules and directions given by coaches.
5. If I can’t participate in group exercise classes by myself, it is the responsibility of my caregiver to work one on one with me, providing supervision and assistance during the class. My caregiver is responsible for filling out and signing the appropriate paperwork at the fitness facility for him/her to assist prior to the start of class.
6. If I am dependent upon a legal guardian, my legal guardian or caregiver must come to the Sunday Strong Class with me and sign and comply with Parts C and D (below) in order for me to be allowed to participate. My caregiver may only leave the property if, after asking, they have been given special permission from a coach to leave.
7. A coach has the right to ask me to leave class if I do not follow the rules, if I am not being safe to myself or others, and/or if I am not able to follow along with the group instruction.
8. Sunday Strong has my permission to use my likeness, photo, video, name, voice, and words for publicity and fundraising purposes in any medium of its choosing.
9. Sunday Strong has collected my personal information on this form. This will be used to create an "Athlete Profile" that will be shared with any Sunday Strong Facility and Coaches where I participate.

As a legal guardian/caregiver of a Sunday Strong Athlete, in addition to those set forth above in Part B, you agree to the following terms and conditions:

1. Always monitor your athlete from the waiting area designated by the coach or facility, unless the coach has specifically given you permission to leave. If you do leave during class, promptly return prior to the end of class for pick up. Coaches/Directors cannot be held responsible to wait with an athlete until a guardian arrives.
2. If your athlete can’t participate in the group exercise class by themselves, it is required that you work one on one with them, providing supervision and assistance during the class. It is your responsibility to sign the appropriate paperwork for the fitness facility to assist prior to the class.
3. Classes and the use of the facility are for registered Sunday Strong athletes only. Please do NOT participate in any class or use the facility or equipment unless you have been given special permission by the coach or other authorized representative of the facility. If you do participate or assist your athlete with permission, you hereby assume any and all risks for your participation and hereby waive, release and discharge Sunday Strong from any liability related in any way to your participation.
4. Monitor your athlete’s behavior throughout the class to ensure it is appropriate, safe to themselves, and safe to others, and that he/she follows all rules set by the coach and host facility.
5. Prior to an athlete’s participation in any class, caregivers/legal guardians must disclose and discuss with the coach any health concerns, conditions or limitations that could negatively affect the athlete’s ability to participate or affect the health/safety of others.
6. Sunday Strong Coaches and/or Directors reserve the right to refuse any athlete from participating if any of these terms/conditions are violated.
